You will need to provide the following information for reference checks:


  • Details of employment history

  • GP name and contact details

  • Last employer’s name and contact details

  • Declaration of medical fitness


The same needs to be supplied for all partners in the partnership.


Financial viability


You will need to prove you have adequate financial resources to provide the services you are applying for. This can be a statement letter from a financial specialist such as a registered accountant.


Registration form


You then need to fill out the right registration form. This varies depending on the services you plan to provide. You will need to fill in the registered manager with a statement of purpose in place. Be sure not to leave out any details.
